In old times we got extra dmg in attacks and skills walking forward while attacking and less defense, the same defense and less attack walking to the sides or backward.
In 6.x versions in retail these bonuses are the same, more dmg in skills and in normal hits walking forward and less of both walking in another direction.
In notaion apparently the bonus is only for normal hits and not for skills. Fixing this can be the solution about the mega healers classes about being immortal in 1v1, specially against classes without silence.
